Family Participation Form

Hello St. Perpetua Parents,

As a member of the Parent Association, my volunteer job is to keep track of the volunteer hours you report and to bill families for incomplete hours. Here are some frequently asked questions:

How many hours do I have to donate?
15 hours for Kindergarten-only or single parent families. 30 hours for other families. TIP: let me know if you came mid-year (your hours are pro-rated) or are a single parent when you report hours.

What is the charge for incomplete hours?
A charge of $50 per hour will be added to your tuition statement on June 1st, 2006 (not the last day of school). If you owe 30 hours, you will owe $1,500.

How do I report the hours that I donate?
Tell me your family name, how you volunteered, dates and the number of hours:
1) Using the back of the tuition form you receive in the Wednesday folder.
–OR- (don’t do both for the same activity)
2) Email me at: mayrd@comcast.net

How do I do a buyout if I can’t volunteer?
Send your check for $1,500 with the form on the back of the tuition statement to the office. Write ‘buyout’ on the form and the School Secretary will forward the information to me.

If I sign-up for a big job, do I have to report my hours?
No. Send the form on the back of the tuition statement to the office. Write the name of your big job and tell me you will exceed your 30 hours. Big volunteer jobs include: Scout leaders, CYO coach or assistant, Room Parent, etc.

How do I know what my balance is?
You will receive a statement in January and April.

What cannot be included in hours?
Yard Duty, LMYA sports, volunteering at sporting events (only CYO coaches and assistant-coaches can report hours).

Rookie Tips:
1) The last day to report your hours is May 31st. NOT the last day of school.
2) Keep track of what you send me and what I send you in case we don’t match.
Mail gets lost and hours can be totaled incorrectly.
3) I need ALL the information on the form filled out to record your hours.
3) You are the only one reporting your family hours.
4) Don’t wait until after the Major Fund Raiser to find jobs--they will be hard to find.

Annual Major Fundraiser: This popular party is the parents’social event of the year. Call months ahead of the event to get a fun job. Contact: Alicia Wilson 299-9033 or Marci Adams 386-0176.

Parish Activities: Carnival, Christmas pageant, etc. Important: there is a maximum of 10 hours that will be credited for Parish activities. Baking goods are limited to 5 of those 10 hours. Children’s choir does not count.

Extended Childcare: Help supervise children anywhere from 3:00-6:00 (tutoring, playing, etc.). Siblings are welcome! Prepare & deliver Panther House snacks (worth 2 hrs. a snack). Contact: Maria White 284-9070.

Student Enrichment Educational Support (S.E.Ed.S): This group organizes Living History Days, Chabot Observatory and Lawrence Hall of Science, art/math/science programs at school. Contribute your creativity or manpower. Contact: Marilyn Sibley 283-6405.

Friday Faculty Lunches: Cook and drop off a lunch item for the teaching staff (2 hours per item). Contact your Room Parents.

School Office Assistance: Relieve School Secretary from 11:45 a.m.-12:30 p.m. doing small tasks and answering the phone. Contact: School Secretary, Liz Ladoucer 284-1640.

Field Days: Assist with games and activities during the last week of school (great way to be with your kid AND volunteer). Contact: Gerard Richardson 284-1640, #155.

Dance Festival: Preparation, set-up and clean-up for Special Person Day in May. Contact: Shawna Zimmerman 686-6081.

Cooking and Baking: Panther House snacks and faculty lunches are 2 hours apiece. In general: 1-2 doz. Cookies, 1 cake or 2-3 dozen cupcakes = 1 hour apiece.

Classroom Activities: Your teacher needs your help! Take home work is available. Contact your Room Parents. Anne Marie See 253-9147 or Julie Sabbadini 957-1785 know your Room Parents.

CYO Sports and Cheerleading: Assigned Coaches and assistant coaches only.

Scouts: School coordinator, Scout leader or Co-leaders only.
